Module 2: Serious Illness

2.1 Primary Care Topics in Outpatient Palliative Care Part 1: Heart failure, Kidney Failure, Liver Failure, and Progressive Lung Diseases

• Review basic diagnosis and classification of the following categories of serious illnesses: heart failure, end
stage renal disease, end stage liver disease, COPD and other progressive lung disease.
• Recall general principles of primary care management for these conditions.
• Use common tools to estimate prognosis for patients living with these conditions.

• Review basic diagnosis and classification of the following neurologic conditions: dementia, Parkinson’s
disease, ALS, MS, and stroke.
• Describe general expected trajectories for these illnesses.
• Recall general principles of primary and palliative management for these conditions.

• Become familiar with common advance care planning documents
• Understand important terms in advance care planning (Power of Attorney, Advance Directive, Living Will)
• Apply Georgia state law and institutional policy to medical decision-making and surrogacy needs
• Apply ACP in the outpatient palliative care setting

• Understand the factors that influence prognosis in seriously ill patients
• Apply prognostic tools to common conditions seen in palliative care clinic
• Evaluate the utility and the limitations of the prognostic tools presented

• Recognize the structure and various roles of the outpatient pediatric palliative care interdisciplinary team.
• Describe general symptom management approaches in children with serious illness.
• Differentiate opioid and non-opioid pain management in pediatric medicine.
• Identify integrative and non-pharmacologic strategies in pediatric palliative care.
• Discuss death and dying, EOL care, and advance care planning in pediatric patients.
